How do you know if you have transferred from RRMS to secondary progressive?

For me, after having MS For 23 years, I have stopped having obvious relapses but continuing (slowly) disability. 2 years ago, I noticed that I hadn't really had any relapses but I was feeling like things were progressing. My Neuro agrees that I've moved to SPMS. MRI's show no new lesions, but more shrinking and black holes. I now have an electric chair for shopping as I become too fatigued and have a lot of pain with it. My memory is worse. I have greater reactions to heat than before (and it was debilitating then) and now cold affects me in a painful way along with the heat.

Lori Foster. He really isn't a good Neuro in managing symptoms and pain only wants to push disease modifying drugs, which I refuse. I'm stuck with him for now as he's the only one who takes my insurance for now.
